Doctor365 is based on the philosophy of the Swiss clinic, which is genuine care knows no boundaries. Wherever a patient is, whether traveling on business, on vacation on the other side of the globe, or in countries with limited access to quality medicine, he or she always feels the support of the personal doctor. The experience of the traditional Swiss Emergency Clinic is realized online. Thus, certified specialists are available around the clock and in the patient’s native language. In addition, the patient can request a second opinion if they have to go to another clinic or if they cannot find their usual medication.

The main goal of the online project is to give the patient confidence and comfort. The platform features instant communication with the doctor, a clear interface in any language, reliable recording, payment and video consultation mechanisms. In addition, patients can go directly to a specialized doctor, without unnecessary steps with a general practitioner, which speeds up diagnosis and the start of treatment.

We developed Doctor365 as an online medical software that allows patients and doctors to stay connected 24/7. Key features of the platform include:

– Timely consultation and appointment through an interactive calendar. Notifications to remind patients of scheduled appointments.

– Requesting a second opinion when a patient goes to another clinic or when familiar medications are unavailable. The system quickly redirects to Swiss Emergency Clinic experts.

– Direct access to subspecialists without unnecessary steps with the general practitioner. Patients receive a consultation with a specialized doctor instantly.

– Electronic prescriptions doctors write prescriptions directly in the interface. Patients receive the document by e-mail or in their personal cabinet.

– Multilingual support with automatic language selection depending on location and user preferences.

– Consultation status monitoring for patients and doctors. They can see the steps of the process from booking to session completion.

This functionality makes Doctor365 a reliable companion for patients anywhere in the world and ensures doctors have full control over the quality of care.

Client needs

The owner of Doctor365 needed a telemedicine solution that patients could use without long wait for a consultation and language barriers. It was important to him that patients and doctors could communicate directly with each other without intermediaries. Second opinions were needed quickly when patients went to other providers or had a shortage of familiar medicines. In addition, the system had to support emergency consultations in the most remote parts of the world. At the same time, it had to store a complete catalog of appointments, prescriptions and payments. Corporate partners in hotels and airlines were looking for a lightweight widget to support their brand with medical services. First and foremost, every piece of data had to pass regulatory compliance for patient privacy. In addition, the solution was based on a global system of certification and verification of doctors. Specialists from anywhere in the world go through a multi-step KYC process to upload their licenses, diplomas and professional experience, after which their profiles are displayed in a directory with ratings and real-time availability, so that patients are only ever connected with a trusted and verified expert.

Discovery phase

At the start of our cooperation, we dived into the reality of Doctor365 and found out the main barriers to quality telemedicine. We documented every pain point from “I can’t find a dermatologist in my language” to “I never know whether my insurance covers this consultation” and designed it in Business Process Model and Notation.

We identified the needs of three key roles, namely patients, doctors and corporate partners. And built detailed Customer Journey Maps for each type of user. We took a detailed look at the process of patients searching for medical care in a foreign country. Remote residents, tourists, and corporate travelers faced challenges with unfamiliar interfaces, long waits, and language barriers. For patients, it turned out that the main pain point was switching between different services when searching for a doctor, downloading reports and making payments.

At the same time, doctors faced routine difficulties with fragmentation of records between different EHRs, multiple entries of the same data and unstable video and chat tools. Difficult scheduling systems and disparate appointment records distract from clinical work. 

At the same time, there was a request from hotel and airline managers to integrate a telemedicine service as part of a room booking or in-flight system. They demanded an easy way to embed consultations into their own digital channels, while maintaining the brand and not overwhelming users.

As a result of the Discovery Phase, we had clear scenarios formed:

– A patient (remote, tourist, or corporate guest) wants a single action to select a doctor by language, specialty, and cost, then instantly pay and receive a consultation without leaving their app.

– The doctor needs a single dashboard where they can check the patient’s profile, view their referral history, and conduct video or text sessions without constantly switching between different systems.

– A partner (hotel or airline) expects a ready-made white label component that can be easily integrated via JavaScript widget or REST API, while maintaining full transparency of service consumption and automatic reporting on SLA and costs.

Result & Technical Implementation

Based on Discovery, we designed an interface for each user, where key actions are available in a couple of clicks. Implementation via Next.js with SSR/ISR ensured instant page loading, and i18next coupled with dynamic translations eliminated language barriers. To make Doctor365 part of the healthcare ecosystem, we provided full interoperability with existing EHR/EMR systems. Using REST, we synchronized patient, appointment and prescription data in real-time. Thanks to Redis cache, frequently requested doctor filters and schedules are now available without delay.

For patients, we implemented a single client web and mobile portal. Side Rendering and Incremental Static Regeneration server for instant loading of pages with filters by language, specialty, price and nearest available slot. The user can drag and drop the report into a drag and drop area, the download runs in the background and progress is saved when the connection is disconnected. All data is merged into one FHIR compliant patient record, including payment history via Stripe Elements and webhooks that instantly update balances and generate e-checks.

Doctors get access to a private dashboard where schedules are synchronized with external EHR/EMRs via HL7 FHIR protocol. We added the ability to choose the reception mode – live video chat via WebRTC or asynchronous “second opinion” with tabular review of feedback from multiple peers. On the server side, Java handles REST queries, and a Redis cache holds doctors availability data and popular filters, allowing the doctors dashboard to open in milliseconds. Doctors undergo KYC verification in a secure module, uploading licenses and documents that are stored encrypted.

Partners connect to the platform through REST integration. Branding, localization and billing model parameters are configured in the widget. For partners we have developed a separate back office with reports on service usage, SLA metrics and cost analytics, the design of which is adapted to corporate Guidelines.

Thanks to containerization in Docker and Kubernetes, our platform scales automatically as the load grows. For security and compliance with GDPR/DSGVO and Swiss law, we implemented end-to-end encryption, role-based access policies and regular pentests.

As a result, Doctors 365 has become a seamless ecosystem where every patient request, every consultation and every partner integration runs in a single, secure, and high-performance flow.
